Registration
Westside Regional Medical Center strives to insure that your visit with us is a pleasant experience. We trust this information will help to familiarize you with our registration process and procedures before the time of your scheduled visit. One of the many conveniences we offer at our hospital is the complimentary valet parking services available upon your arrival. The valet stand is located at the Admitting entrance, on the east side of the building. Operation hours are Monday through Friday from 7:30 am to 5:30 pm.

All scheduled patients will receive a pre-registration phone call requesting demographic and insurance information. In addition, an appointment will be scheduled if a pre-operative meeting or testing is necessary.
Upon arrival, your information will be verified and you will be requested to sign two forms; a "Consent to Treat" and a "Consent to Bill" for the insurance company. Medicare and Champus recipients will need to sign the additional appropriate forms.
Please bring the following items:
- All currrent health insurance cards
- Picture ID
- Prescription for any tests
- A referral from your physician, if applicable
- Living Will, only applicable if you are undergoing surgery or will be an inpatient.
Registration Checklist:
- Verify with your physician's office that they have obtained the necessary authorization for your tests or procedures. The physician's office must initiate the process of authorization.
- Contact your insurance company to receive pre-certification procedure requirements.
- Contact your physician's office to verify that notification to the insurance company has been completed.
Admitting
Registration is located through the Outpatient Entrance which is on the east side of the hospital.
